How Each Method Works
Non-incisional (buried suture) creates the crease without cutting the eyelid. The surgeon makes several tiny micro-holes along the intended fold line and passes a fine thread through them, forming the crease with sutures. Because no tissue is cut or removed, swelling is minimal and there is no visible scar.
Incisional creates the crease through a continuous incision along the fold. This lets the surgeon remove excess skin and fat and secure a firm, well-defined crease. It is more involved, but it addresses conditions that sutures alone cannot.
Partial incision is a middle-ground option — a few small cuts rather than one continuous line — offering some fat access with faster recovery than the full incisional method.
Ultimately, this is an anatomical decision, not a matter of preference alone. A method that produces a beautiful result on thin lids may loosen quickly on heavy ones.
What to Expect at a Consultation
A surgeon assesses your eyelid thickness, skin elasticity, fat volume, any existing asymmetry, and your desired crease height and shape (commonly described as in-line, in-out, or out-line). The method should be chosen together with the surgeon based on that assessment — be cautious of any recommendation made without examining your eyes.
