Facial contouring is the surgery most associated with the "Korean look" — the smooth, balanced lower face and soft V-shaped jawline seen across K-pop and K-drama. It's also one of the most technically demanding categories of plastic surgery, because it reshapes the actual bones of the face. This guide explains the main procedures — V-line jaw surgery, cheekbone (zygoma) reduction, and square-jaw reduction — who they suit, the real risks and recovery, and why the surgeon's skill matters more here than almost anywhere else.

What "Facial Contouring" Actually Means

Facial contouring is a family of bone procedures that reshape the jaw and midface for a slimmer, more harmonious profile:

  • V-line surgery — reshaping the chin and jawline into a narrower, softer V. Often combines a genioplasty (chin reshaping) with jaw-angle reduction.
  • Square jaw reduction — shaving or cutting the mandibular (jaw) angle to soften a wide, angular lower face.
  • Cheekbone (zygoma) reduction — repositioning the cheekbones inward to narrow a wide midface.
  • Chin surgery (genioplasty) — advancing, reducing, or centring the chin for balance.

These are frequently combined, because true facial harmony comes from the jaw, chin, and cheekbones working together — not from one bone in isolation.

The Main Procedures Explained

Most contouring plans draw on a few core operations:

  • Jaw-angle (mandibular) reduction. The flared, angular part of the lower jaw is reduced through the mouth (no external scar) for a smoother line from ear to chin.
  • V-line osteotomy. The chin bone is cut and narrowed — and often the whole lower jaw reshaped — to create a defined, tapered point. This is the signature "V-line" result.
  • Zygoma (cheekbone) reduction. The cheekbone body and arch are cut and moved inward and secured, narrowing a wide or high-projecting midface.
  • Genioplasty. The chin is moved forward, back, up, or down to balance the profile — sometimes instead of an implant, sometimes alongside jaw work.

Who Is a Good Candidate?

Contouring is bone surgery, so candidacy is about your anatomy and your goals:

  • A wide or square lower face from prominent jaw angles.
  • A wide midface from projecting cheekbones.
  • A short, receded, or long chin that unbalances the profile.
  • Realistic goals and good general health. This is major surgery; the aim is harmony and proportion, not an extreme or cartoonish taper.

Just as important is who is not a good candidate: if the fullness is soft tissue or fat rather than bone, a lighter approach (lifting, liposuction, or fillers) may achieve the look without cutting bone. A good surgeon will say so.

Surgical vs Non-Surgical Contouring

Not every "V-line" wish needs an operation. It helps to separate the two paths:

Surgical contouringNon-surgical contouring
AddressesBone (jaw angle, chin, cheekbone)Soft tissue, fat, mild sagging
ExamplesV-line, zygoma reduction, genioplastyBotox (masseter), filler, thread lift, Ultherapy
ResultPermanent, structural changeTemporary, subtle refinement
DowntimeSignificant (weeks of swelling)Minimal to none
Best forGenuinely wide/angular bone structureSlight fullness, early aging, "trial" softening

Masseter Botox and jawline fillers can slim or define a face impressively when the issue is muscle or volume rather than bone — a far smaller commitment than osteotomy.

The Real Risks and Recovery

Because contouring reshapes bone near important nerves, the honest picture matters:

  • Swelling is significant and long. The lower face swells dramatically at first; most subsides over a few weeks, but the fine final contour can take several months to fully settle.
  • Numbness. Temporary numbness of the lip, chin, or cheek is common as nerves recover; lasting numbness is a rare but real risk in inexperienced hands.
  • Asymmetry and over-resection. Removing too much bone, or unevenly, is difficult to correct — which is why precision and experience are everything.
  • A demanding early recovery. Expect a soft/liquid diet, a compression garment, and limited jaw movement in the first weeks.

Tip: Contouring is defined as much by restraint as by reduction. Over-aggressive bone removal can leave the face looking gaunt or unstable as you age — natural proportion outlasts an extreme taper.

Why the Surgeon Matters More Than Anywhere

Cutting facial bone safely, symmetrically, and near major nerves is among the most advanced skills in plastic surgery. The gap between a specialist and a generalist is wider here than in almost any other procedure — and mistakes are hard to undo. Prioritise a surgeon with deep, specific facial-contouring experience and before-and-after cases that look natural and balanced, not extreme. The principle is the same one we stress throughout: choose the master specialist before the price. If you're planning nose work in the same trip, see the complete rhinoplasty guide, and for how difficult bone cases are approached, our guide to complex revision rhinoplasty shows why experience is decisive.

Frequently Asked Questions

Is V-line surgery the same as jaw reduction? They overlap. Jaw reduction shaves the angular jaw angles; V-line surgery goes further, often cutting and narrowing the chin bone (and reshaping the whole lower jaw) to create a tapered point. Many V-line plans include jaw-angle reduction.

Will I have visible scars? Usually not. Jaw and chin contouring is typically done through incisions inside the mouth, leaving no external scar. Your surgeon will explain the specific approach for your case.

How long is recovery? Plan for significant swelling in the first two to four weeks, a soft or liquid diet early on, and a compression garment. Most people look presentable within a few weeks, but the fine final contour continues to refine over several months.

Can I get a V-line look without surgery? Sometimes. If the fullness is muscle or fat rather than bone, masseter Botox, jawline filler, thread lifting, or skin-tightening can slim and define the face with little downtime. If the structure is genuinely wide bone, surgery is the only way to change it.

Is facial contouring safe? In experienced, specialist hands with proper planning it is a well-established surgery — but it reshapes bone near facial nerves, so the surgeon's skill is the single biggest safety factor. This is not a procedure to choose on price.
